3) US is #11 in Education (a fall from 2012)

I wonder how they determined the rankings for Health. We aren't number one in life expectancy. What other metrics would they consider?

Good question.

There are many metrics that others have used, including (as I mentioned) maternal mortality, "patient satisfaction," and other "soft" measurements.

In this study, they measure infrastructure, basic health outcomes (both objective and subjective), and preventive care.
