HOLY CARP!!!
I'm on vacation (well, staycation) from work next week. I was asked to come in for an hour or two this morning to help with pre-op inspections as only 5 people are trained on this.

My vertigo hasn't really returned since those bad 2-3 weeks in July. I've still been terrified to sleep on my left side, as uncomfortable as it is always sleeping on my right (as I have left side BPPV, ie. vertigo) I had vague plans of maybe trying sleeping on my left during my week off work.

So I wake up early this morning, drive to work, and I'm standing at the break room sink drinking some water. As I tilt my head back to take a drink of water, it suddenly feels like the room shifts and spins around. I grab the counter for balance, and several thoughts run through my head.

1. No. Not this shit again.
2. Good thing I'm off work the next week.
3. This better not ruin my vacation. (I have 2 weeks vacation per year. I used my other week in April and spent half the week with bad food poisoning. I was glad I didn't have to miss any work for it, but on the other hand, it kinda sucked wasting some of my limited vacation time.)

I've been feeling slightly dizzy ever since this happened, but not incapacitatingly so. This happened at 4:30am, it is now 5:45am. I was planning on working a few hours since I was there anyway, but since this happened, I left as soon as I finished pre-op which took me about 45 mins.

Many years ago I was diagnosed with Meuniere's syndrome in one ear. Early on I had a few episodes of sudden, brief, very violent vertigo. My doc explained -- IIRC -- a section of canal in my inner ear was too narrow, and when it became congested it swelled and brushed up against the eighth cranial nerve, which controls direction or directionality.

I can't remember if your doc examined your ears, but this may be something to think about.

Just anecdotal but every single person I know IRL who has suffered from BPPV has also had thyroid problems. Several were already on meds when the vertigo hit and supposedly their thyroid numbers were in range while others (including myself) were diagnosed shortly after their first BPPV episode. I can't imagine why thyroid would cause BPPV but I did find reference to vertigo being one of the potential outcomes of untreated or improperly treated thyroid issues in a few of the books I have read. So if you haven't had your thyroid tested, it might be worth exploring. I can't imagine what thyroid has to do with vertigo, but I'm talking like 10 people I know IRL with vertigo and every single one also has thyroid issues....for whatever that's worth.

In the meantime you may already know there's all kinds of head positioning exercises you can do to clear the vertigo up. I had good success with the Epley Maneuver but there are many others which a quick consult with Dr. Google can help you with. Good luck - I know how much it sucks so I hope it stays away for you!

This is difficult stuff, Riley. I don't have any advice other than to get checked again to see what is causing it this time. It could be something different than last time, or the same. Either way, it makes it tough to get around.
