George, Bachophile - have you ever participated in a lung transplant? I understand that it is a quite sophisticated procedure. In all of Germany, there are only two hospitals that perform lung transplants.

So happy to hear this. It's been a long wait for him and his family, and yes, prayers for him, his family, and the donor's family. From the donor's family's perspective, they can find at least some relief from their anguish to know another person and family will benefit. Having been part of a donor family, I can say this is a very good thing during their grief. |

When lung transplants became a thing, in the mid 80s, our place started a program. I was a member of the heart surgery team, so I knew all the surgeons well. However, only one of the five of us actually gave anesthesia for the procedures (he had attended some courses, etc). It is a terribly complicated procedure (unlike a kidney transplant, which has become almost trivial, or a heart transplant which is difficult, but not complicated). There are multiple things to hook up to each other, and the fit has to be right. God help you if the suture line is not perfect and things start to leak - either air or blood. The other issue is that many of the patients who need a lung transplant have coexisting heart disease (heart failure). This can complicate recovery in a major way. Jon's been going to the hospital every few months to check on the status of his heart, and my understanding is that this is not an issue for him. I don't know current protocol, but they may put him on a heart lung machine. However, if his heart is strong enough, and one lung is good enough, they may work with just one side, and then switch over and do the other. Regardless, I'm sure that they'll be prepared to put him on bypass if things go south. The recovery, I'm sure will be prolonged. However, he is young, with no other diseases complicating the issue, so with luck, he'll do ok. He will probably spend a couple of days on a ventilator. The amount of plumbing that will be inserted is epic. He and his family will know that he's getting better as the number of tubes starts to decline. Be sore as hell for a long time, but that goes away. You can read more about Duke's program here. And you can read more about how the procedure is done here (Johns Hopkins link).
