Medical emergency. He helped big time til the ambulance arrived. I'm sure he'll chime in later. I don't know any details. |

Sounds like some free train rides should be forthcoming. |

The Confederate soldier was peculiar in that he was ever ready to fight, but never ready to submit to the routine duty and discipline of the camp or the march. The soldiers were determined to be soldiers after their own notions, and do their duty, for the love of it, as they thought best. Carlton McCarthy | |

Funny thing about being a doctor. Even when you're off, you're never off. Other people retire. Doctors don't. Doctors die as doctors. For the good ones, it's not a vocation, it's a calling. Thank God for the good ones... |

"Is there a doctor on board? If so we could use your assistance in car 631 in the lower level." Seems some old guy who is on Pradaxa (a nasty blood thinner), upon nearing Truckee California, developed a serious nosebleed. Not much I could do, so I iced it, put pressure on it and reassured him and his wife. Well, Charlie's nosebleed would not stop, and he had lost a couple of hundred ccs of blood. The conductor asked me what to do, and I told her that it was really Charlie's call. We were about half an hour out of truckee, with possible delays due to freight traffic. I said that if it were me, I'd get off ASAP and go to an ER. They agreed. So, they backed the train up about a quarter of a mile where the paramedics met us at the crossing. They gott Charlie and his wife on the ambulance and they went off to the nearest ER where he could get proper treatment. No big deal, and I really didn't do anything. There was, however, a LOT of blood. |

George, as a doctor, is there any potential legal liability for you in those sorts of situations? |

Good question. There are Good Samaritan laws that protect you, though if someone wants to sue, they will. I chose to be very conservative in my recommendations to these people always saying "it's your call." If I had suggested that we go onto the next stop, things could gotten dicey if we got delayed. Like now, when were sitting behind a fright that has "issues." We are delayed for an hour, sitting in the desert. If Charlie had kept bleeding I the middle of nowhere, things could turn sour. I'm glad he's off the train. |

And then..... Our sleeping car attendant came back to us and said that someone else was sick, he had a fever and felt horrible. Of course, he called because he knew that there was " a doctor in the house." This guy was running what felt to me to be a low grade (<101) fever. He had a rattly cough. He asked if he should take some aspirin and if I had any. My response was, "Well, I really can't give you advice without examining you, and, no, I don't have any aspirin to give you. If you have some Tylenol,, take it. If you're too sick to proceed, get off at the next stop, Reno, and see an ER. |

Nice work George. I was eating in a restaurant one time with my SIL who is a volunteer medic - he rides in the back of an ambulance. A guy standing near us keeled over out cold on the floor. The manager came running over and was really panicked. The SIL was hesitant but then decided to lend a hand since he deals with this sort of thing. He didn't do much but he figured out that the guy was breathing and calmed the manager down while waiting for an ambulance to show up. The manager picked up our tab. |
