Sometimes, I see something that makes me pause and say, "There but for...." Had a case this morning. Betty is 55. Last year her husband of 31 years UAD'ed (Up and Died) of a heart attack. It was sudden, with no warning. Just UAD'ed. So, she thought she should get checked out for anything. Well, her doc found that she's a diabetic, and a pretty severe one as well. Her diabetes has caused a bad neuropathy, so she can't feel her feet and toes. Of course the fact that she smokes 1/2 pack a day doesn't help the blood flow to her feet, and adds to the diabetic blood flow problems. He felt her abdomen, and noted that her uterus was too large for a woman her age - uterine cancer. So, about 8 months ago, she had a radical hysterectomy, and went through chemotherapy for it (still in treatment), losing all her hair. About 3 days ago, she was admitted with a non-healing ulcer on the bottom of her left foot. Sure enough, it's a diabetic ulcer. Infected and nasty. The podiatrist thought that it was getting worse by the day, and this morning we brought her to the OR to clean out the ulcer (ulcers, actually, she also had an abscess on the top of her foot), with the possibility of needing to amputate her big toe. I met her in the holding area, and after my assessment and explanation of what I'm going to do, we took her to the holding area. She's a delightful lady, with a great sense of humor. Case took about an hour, and the podiatrist didn't have to amputate the toe; he just cleaned out the wounds - though he's not optimistic on the long-term survival of that toe. I brought her to recovery, and she woke up really nice, smiled and opened her eyes when I asked her to. "What did Dr. M do?" "He cleaned up your wounds and ulcers." "I still have my foot!" "Yes, Betty, you do." How bittersweet. |
