not the biggest one ive done, but respectable size.

actually easier then expected. it was a low grade tumor not invading other structures, just pressing on everything. it was situated in the right lumbar region pushing the right kidney medially past the midline. my only real concern was the right adrenal and its vein feeding directly into the cava, but it peeled off quite nicely.

went into the room at 8, finished by 11. no major blood loss and no need for blood transfusion.

symptoms were only vague abdominal discomfort and the feeling that the guy was gaining weight and getting fat. he got that right.

all in all a satisfying case.

sarcomas generally do not invade contiguous structures, but the higher grade they are, the more they tend to create parasitic blood vessels which create a web of doom that surronds them, so that any non careful handling will cause considerable bleeding.

they metastasize hematogenously, usually to the lung.

rarely one can find sarcomatosis, which is when the abdomen is full of these little daughter masses all over the place, usually rapidly fatal.

the low grade liposarcs grow slowly and insiduously, with about a third to one half eventually, after a number of years, transforming into high grade, or dedifferentiating into an anaplastic catastrophe. so its best to get them out ASAP.
