Kid comes into the ED last night. He's got an abcess on his buttocks that covers a large portion of the right side. Temp is 99.8 and he's been throwing up. Doc orders a CBC upon presentation and the WBC is 21.5. So ok-dokey, let's culture and treat the abcess, put the kid on an OTC for pain and prescribe some antibiotics. I didn't expect the MD to order throat swabs for flu and strep. His rationale? Kid's febrile and his throat is red. Definitely CYA medicine. Thank you, ambulance chasers... |

1. Are you sure it's CYA? 2. Are you sure it's only CYA? 3. Are you sure it's due to ambulance chasers? 4. Are you sure it's only due to ambulance chasers? Other possibilities: 1. Could you be wrong to think that there is no medical reason to order a throat swab? 2. Could the MD simply be inexperienced, incompetent, both, or one who simply makes a bad call every once in a while and this one is just one of those bad calls? 3. Could the MD and/or his associated organization simply be acting to maximize revenue in a pay-by-service incentive system? (e.g., the more services rendered, such as ordering tests and interpreting test results, whether needed or not, the MD and/or his associated clinic/hospital get paid more.) |

1. The kid's throat is read because he's throwing up. He's throwing up because of intense pain from a monster abcess. I know this. The M.D. knows this. Any medical personnel with a mouth-breather I.Q. knows this. Ain't no pustules(strep), pardner. And the fever is low grade (R/O flu). 2. Yep. See #1. 3. Yep. But since the initial physical presentation shows a red throat, it's CYA. Kid has an abcess, probably staph...but if it's a strep and it goes worst-case systemic, some sorry-ass Perry Mason wannabe will be standing in line to sue the hospital, the ED physician and the parking lot attendent, because little Johhny now has a valve problem...stemming from his strep throat he never had. 4. You're damn right I am. 5. The bill for the strep was $151.00. The Flu A/B was $160.00. The hospital won't be reimbursed that, since the kid was LaCHIPS, but I hope very dime of that comes out of your pocket, not mine. |
