Finished up in the OR at about 4 + 4 (that's 8). Hit the sack at about 10, and at Midnight (that's 4 + 4 + 4), OB called me to put an epidural into a lady who was 4 centimeters dilated, having her 4th kid. I got her comfy, having threaded the catheter about 4 cm into her epidural space at the L3-4 interspace. Fourtunately, it was an easy stick, she only weighed about 4 + 4 + 4 stone. I got back to bed and tossed and turned for about 45 minutes, at least the last time I looked at the clock it was about 1:30. At 4 AM I got a call from the ICU that there was a lady in bed four with a dissecting thoracic aortic aneurysm. She was 86 years old, and it was a huge aneurysm, about 4 cm in diameter. It took about 45 minutes for everyone to get there, and at 5:30 we started. I put in my four lines (2 iv's, arterial line, and neck line) after I got her off to sleep. Fourtunately, there were only four rooms scheduled today, instead of the usual five, so there was no reason to fource another surgeon to wait while the aneurysm was done. My partner came in to relieve me at 7, and I left. This lady has had no serious issues in her past history, other than a mastectomy for breast cancer about forty years ago, and she took only aspirin and three vitamins. She is a tiny little thing, weighing about 44 kg. Hopefully she'll do well. This operation has a 10-20% mortality. Not operating has a 100% mortality. I'm a tired dog, but I have a nice 3 day weekend to recover (don't tell Mr. Orzag, 'kay?) |

It's astonishing to me how routine these aneurysm repairs seem to be. My grandmother died of one in 1965 at age sixty-four. There was little they could do for her. |

Just checked. They got done about half an hour ago. Evelyn survived the surgery. Now, we'll see if she survives the postop course. |

Fascinating story. Last night (at 4AM) I didn't hear much of the history on Evelyn. I was just reviewing her chart and found out EXACTLY what happened. She went to her doc's office yesterday afternoon complaining of symptoms consistent with a urinary tract infection. He asked her to provide a urine specimen, and while doing that, she fell to the floor in the washroom, passing out. She was sent to the ER, where she underwent the usual workup for someone who passes out (carotid artery evaluation, EKG, and a CT of her head - all of which were negative - sometimes, 86 year old ladies faint). Blood work revealed a result which was consistent with, but not diagnostic of, a blood clot to her lungs (pulmonary embolus). So, the next step was to get a chest X-Ray which revealed the presence of the aneurysm. Some time during her ER stay, she did have a fleeting episode of chest pain consistent with her diagnosis, but she said that if she had not been in the hospital, she would have written it off as "old lady pain" and thought nothing of it. Before you know it, she was on the table. She's doing fine, by the way. |
