When a match post has been ended with a submission, can the person being put into the hold get himself out or does the ref have to do it for him?

It's the referee's decision. If the ref doesn't post for at least 12 hours, then you can go ahead and continue forth with the match yoursef.

Normally, a referee will determine whether or not your opponent submits, and if that's what you want, it's best to put some indicator at the end of your post so there's no confusion. If your just putting your opponent in a submission to start their next post off with (which I've found fun to do), then just perhaps put that at the end of your post.
If a referee though, doesn't post in 12 hours of your post, your opponent may then go ahead and bypass the referee and break the hold (or submit, if forfieting your own match has become cool and hype recently) himself.
That's how I understand that it works, and I hope it helps in some way.

"1) Referees are designated to a match for a reason. Unless a match has been planned out by all participants, DO NOT count your own pins. If said ref has not posted in a TWELVE hour period from your opponent's last match, only then may you continue your match. However, you MAY NOT continue the match if the opponent ended their post with a finishing maneuver, because those counts should be on the Ref's shoulders."
After 12 hours, its safe for him to post a ropebreak, or breaking of the hold. Not only that but im pretty sure with consent from Both or multiple parties to keep the match rolling, you can break the hold
